Position Summary
- We are seeking a compassionate, organized, and motivated Case Manager to support adults with developmental disabilities in achieving greater independence and quality of life. You will coordinate services, track progress, support staff, and act as a key advocate for your assigned clients.
Roles & responsibilities
Key Responsibilities
- Develop and monitor Individual Service Plans (ISPs)
- Review assessment data and track client progress
- Coordinate and attend interdisciplinary team meetings
- Prepare progress reports and maintain accurate case records
- Train and support direct care staff on goal implementation
- Provide job coaching and community field support as needed
- Ensure compliance with confidentiality and regulatory standards
- Serve as liaison between clients, families, and service providers
- Additional duties as assigned
Qualifications
Required Qualifications
- Bachelor’s degree (Psychology, Human Services, Sociology, Social Work, or related field preferred)
- Strong interest in behavioral health, healthcare, or social services
- Excellent written and verbal communication skills
- Strong organizational skills and ability to work independently
- Reliable 4-door vehicle (4–5 seater)
- Valid California driver’s license and current auto insurance
- Clean background check
- CPR and First Aid certification (must be completed within 30 days of hire)
Preferred
- Experience working with individuals with developmental disabilities
- Spanish language skills

50000-52000
