About this job
As a Lead Case Manager at Pacific Health Group, you’ll play a hands-on role in supporting individuals with complex needs through our Enhanced Care Management (ECM) programs. This role goes beyond coordinating care, you’ll build trusted relationships with members, meet them where they are, and help address real-life challenges such as housing, food access, mental health, and navigating healthcare systems.
You’ll conduct frequent in-person visits, coordinate medical and community-based services, advocate on behalf of members, and serve as a consistent, compassionate point of support. Your insights from the field will directly inform how our programs evolve, ensuring we continue to deliver responsive, whole-person care to the communities we serve.
Roles & responsibilities
Conduct frequent in-person visits with members in homes, shelters, and community settings to assess needs and build trust
Develop and coordinate individualized care plans addressing medical, behavioral, and social needs
Arrange and manage end-to-end services, including medical appointments, transportation, housing support, and community resources
Serve as a consistent advocate for members, navigating systems and resolving barriers to care
Maintain regular communication with members, families, healthcare providers, and community partners
Document assessments, care plans, and progress notes accurately and in a timely manner
Monitor member progress and adapt care strategies as needs evolve
Stay current on Medi-Cal, CalAIM, and Enhanced Care Management requirements and ensure regulatory compliance
Participate in ongoing training and professional development to strengthen care delivery
Collaborate with team members and support broader program goals as needed
Qualifications
3–5 years of experience in case management, social services, healthcare, or a related field
Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management (ECM) programs
Strong understanding of healthcare systems and community-based resources
Demonstrated ability to conduct needs assessments and develop individualized care plans
Excellent communication, interpersonal, and cultural competence skills
Strong organizational and time-management skills with the ability to manage multiple cases
Proficiency using case management systems and related technology
Valid California Driver’s License (Class C minimum) with access to a reliable, insured personal vehicle
Ability to work independently in the field and collaborate effectively with a team
Salary
$30/hr
